Membership is for all; be it a principal member or a dependent. The requirement is a Valid National ID for all adult members, minors can be enrolled by their guardians ID Card.

 

Premium

• Premiums are payable weekly, monthly, or annually as ascribed.
• Payment of Premium is subject to an insurance year of 12 months, relative to commencement date.
• Premiums shall be paid continuously without default.
• Default in one-month full payment shall result in an inactive status.
• Reactivation shall take effect 14 days after payment of all outstanding premiums.

Benefits

✅ Commencement – 1st of the third month after registration.
✅ Dental Subsidy - GHC100.00
✅ Spectacles Subsidy (2YRS) – GHC100.00
✅ Maximum Out-Patient visits per month to a hospital will be two (2) times with a maximum cover of GHc300.00 per visit.
✅ Maximum Out-patient limits: Prescription - GHC100.00 MRI – GHC600.00* | CT Scan – GHC300.00* | Myomectomy - GHC1,500.00 | EOU - GHC500.00 (Abortion is not covered by the scheme)
✅ The Outpatient limits per visit apply to all accredited facilities.
✅ Maternity benefit commences after the twelfth month of membership and the maximum limit is GHC2,200.00.
✅ Admission cases are paid to a limit of GHC1,200.00 per admission (once a month).
✅ Pre-existing conditions (known or unknown) requiring surgery or hospitalization shall not be covered in the first year of the insurance cover.
⚠NB: Any additional cost to the stated limits will be borne by the member.

• Dial *988# on any mobile handset to initiate USSD registration.
• Select 1 - Register, then 1 – Premier Care.
• Select a Payment Plan.
• Complete your First Name, Middle Name (if applicable), Last Name, Date of Birth, Location, ID Type, ID Number and Mobile Network.
• Wait for a POP-UP menu on your handset and enter your MOMO Pin to complete first payment.
-For MTN users, should a pop-up menu not show within a minute, kindly check your Wallet for Approval by dialing *170#, select Option 6 (Wallet), then Option 3 (Approvals), and select the respective transaction to … payswitch … to complete payment.
-Dial *928*118# to Complete Registration.
• Your Policy Number and link to Policy document will be delivered via SMS.
• Your List of Service Providers will also be delivered as a link via SMS.
• To access healthcare, produce your National ID and show your policy number as well.
